Azithromycin dihydrate
Indications/Uses
Infections in lower resp tract including bronchitis & pneumonia, skin & soft tissue infections, acute otitis media & in upper resp tract including sinusitis & pharyngitis/tonsillitis. Eradication of streptococci from the oropharynx. Treatment of uncomplicated genital reactions due to C. trachomatis, chancroid due to H. ducreyi & uncomplicated genital infections due to non-multiresistant N. gonorrhoeae. Prophylaxis against M. avium-intracellulare complex (MAC) infection w/ rifabutin. Treatment of disseminated MAC in patients w/ advanced HIV infection w/ ethambutol.
Dosage/Direction for Use
Adult Treatment of STD caused by C. trachomatis & H. ducreyi 1,000 mg as single dose. Susceptible N. gonorrhea 1,000 or 2,000 mg in combination w/ ceftriaxone 250 or 500 mg. Prophylaxis against MAC infections in HIV patients 1,200 mg once/wk. Treatment of disseminated MAC in advanced HIV patients 600 mg once daily. All other indication including upper & lower resp tract infections 500 mg daily for 3 days. Total daily dose: 1,500 mg. Childn (>45 kg) Total dose: 1,500 mg.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic.
Special Precautions
Discontinue use if allergic reactions occur. Risk of hepatotoxicity, superinfection, C. difficile-associated diarrhea, ergotism (w/ ergot derivatives). Renal impairment; myasthenia gravis. Prolonged cardiac repolarization & QT interval in patients w/ congenital or documented QT prolongation; currently receiving QT-prolonging drugs (eg, antiarrhythmics class IA & III, antipsychotics, antidepressants & fluoroquinolones); electrolyte disturbance particularly in cases of hypokalemia & hypomagnesemia; clinically relevant bradycardia, cardiac arrhythmia or insufficiency. Pregnancy & lactation. Elderly.
Adverse Reactions
Hearing impairment (including hearing loss, deafness &/or tinnitus); nausea, vomiting, diarrhea, loose stools, abdominal discomfort (pain/cramps) & flatulence; abnormal liver function; allergic reactions including rash & angioedema; local pain & inflammation at site of infusion. In DMAC prophylaxis: dyspepsia, pruritus, headache, arthralgia.
Drug Interactions
Reduced serum conc w/ antacids. Increased serum levels of P-gp substrates (eg, digoxin) w/ macrolide antibiotics. Possibility of interaction w/ ergot derivatives. Increased conc of phosphorylated zidovudine. Increased Cmax & AUC0-5 of cyclosporin. Increased conc w/ nelfinavir. Risk of neutropenia w/ rifabutin. Carbamazepine, cimetidine, coumarin-type oral anticoagulants, cyclosporine, efavirenz, fluconazole, indinavir.
